29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees.    a) Jobsite: Inside trench, unprotected east end - An employee who conducted water and sewer operations inside a trench, which measured approximately 21' in length, 4' 8" in width, and 8 ft. in depth, without a safe means of egress was exposed to cave-in hazards without safe egress.    Observed on or about 9/21/18.

Observed on or about 9/21/18.  The employer was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number1326082, citation number 1, item number 2, and was affirmed as a final order on 11/1/18, with respect to a workplace located at Broad and Ellworth St, Philadelphia, PA 19147  The employer was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number1323007, citation number 1, item number 2, and was affirmed as a final order on 11/1/18, with respect to a workplace located at 2th and Poplar St, Philadelphia, PA 19130.
