The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head. On or about 9/21/2018 at 3225 Ivy Farm Path, Lot 36, Buford, GA workers were exposed to falls of up to 24-feet to the lower level from the 10/12 pitch roof where the employer failed to ensure that full body harnesses were in use during roofing operations and allowed the use of rock climbing harnesses.
