29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: On 9-19-18, at the Highway 61 bridge construction site, South Fabius River, an employee assisting in hanging brackets and at least one other employee carrying materials at the side of the bridge on the bridge deck, approximately 18.5 ft over the bank, were not wearing fall protection equipment or protected by some other method from a fall.

29 CFR 1926.760(a)(1): Except as provided by paragraph (a)(3) of this section, each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level shall be protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems: On 9-19-18, at the Highway 61 bridge construction site, South Fabius River, an employee connecting I-beams, when obtaining materials from the south access roadway, walked a beam, approximately 21 ft over the river, without fall protection.
