29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) Garage Roof, 438 South Kennedy Drive, McAdoo, PA: Employees were engaged in replacement shingling activities on an 8:12 pitched roof without any means of fall protection, exposing the employees to an nine (9) foot, four (4) inch fall hazard, observed on or about September 21, 2018. In accordance with 29 CFR 1903.19, abatement certification is not required for this violation. The hazardous condition was corrected during the inspection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) Garage Roof Eave, 438 South Kennedy Drive, McAdoo, PA - Employees used a Werner Type 1A Fiberglass Extension Ladder Model D6224-2 for roof access/egress with a left fly section side rail that extended eight (8) inches and a right fly section side rail that extended eighteen (18) inches above the top of the roof exposing the employees to an approximately fourteen (14) foot fall hazard, as observed on or about September 21, 2018. In accordance with 29 CFR 1903.19, abatement certification is not required for this violation as the hazardous condition was corrected during the inspection.

In accordance with 29 CFR 1903.19, abatement certification is not required for this violation as the hazardous condition was corrected during the inspection.
