29 CFR 1926.502(i)(3): All covers were not secured when installed so as to prevent accidental displacement by the wind, equipment, or employees: a. 145 Commons Way, Bridgewater, NJ - Roof The employer did not ensure that floor hole covers were secured to prevent accidental displacement by the employees. An employee engaged in clean-up activities on the roof was able to remove a cover that was not secured and fell through the hole. Violation occurred on or about September 21, 2018
