29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: (Construction Reference: 1926.59)    (a) 6227 Market St., Philadelphia, PA 19139 - Employees were exposed to chemical hazards in that a written hazard communication program was not developed or implemented in the workplace where employees used hazardous chemicals, including:    - Calcium oxide, quartz, and hexavalent chromium contained in Lehigh Hanson Portland Cement; and   - Calcium magnesium tetrahydroxide, calcium magnesium dihydroxide oxide, calcium hydroxide, and crystalline silica contained in Graymont Dolomitic Hydrated Lime.    This condition was observed on September 21, 2018.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (Construction Reference: 1926.59)     (a) 1340 Reed St., Philadelphia, PA 19147 - Employees were using hazardous chemicals on a construction site, but the employer had not provided them with hazard communication training, thus exposing employees to hazards of chemicals.  Hazardous chemicals used on site included:     - Calcium oxide, quartz, and hexavalent chromium contained in Leigh Hanson Portland Cement; and   - Calcium magnesium tetrahydroxide, calcium magnesium dihydroxide oxide, calcium hydroxide, and crystalline silica contained in Graymont Dolomitic Hydrated Lime.    This condition was observed on September 21, 2018.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):    (a) 6227 Market St., Philadelphia, PA 19139 - The employer did not ensure the working level of a fabricated frame scaffold, approximately 8.8 feet above the lower level, was fully planked while an employee stood on a single plank and applied flashing material to the exterior of a building, thus exposing the employee to fall hazards.  This condition was observed on September 21, 2018.

An employee applied flashing material to the exterior of a building at working level of approximately 8.8 feet, thus exposing the employee to fall hazards.
