29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service:  On or about 09/12/2018 at the Big Y Construction Project, an employee was using a lanyard that had a damage snaphook and damaged webbing as part of his personal fall protection system.

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii)  On or about 09/12/2018 at the Big Y Construction Project, employees were not trained by a competent person on the correct procedures on inspecting the fall protection systems in use.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about 09/10/2018 at the Big Y Construction Project, employees were using an extension ladder to access and descend to and from the roof decks and the ladder did not extend 3 feet past the landing.
