29 CFR 1926.102(a)(1): 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  On or about September 24, 2018, and at times prior thereto, one employee was not protected by eye or face protection while using hammers and saws on the construction site, exposing the employees to the hazard of being struck by flying debris.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about September 24, 2018, and at times prior thereto, three subcontracted employees were installing trusses on top of the residential property under construction, exposing the employee to a fall hazard over 6 feet.
