29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:     (a)  Westminster Auto Parts Inc, dba Stadium Auto Parts Inc, at 2323 W Mulberry Place, Denver, CO: On and preceding 9/24/18, the employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  Employees performed operations related to recycling automobile parts, including but not limited to draining liquids from automobile parts, recycling chemicals from automobile parts, using pry bars or hammers to remove axle shafts from spindles, and removing engines and other parts from automobiles.  Employees wore prescription non-safety glasses during these operations or wore no eye protection during these operations.  The employer did not ensure that all employees used appropriate eye protection.  This condition exposed employees to a flying particle hazard.

29 CFR 1910.136(a): The employer did not ensure that each affected employee use protective footwear when working in areas where there is a danger of foot injuries due to falling or rolling objects, or objects piercing the sole, and where such employee's feet are exposed to electrical hazards:  (a)  Westminster Auto Parts Inc, dba Stadium Auto Parts Inc, at 2323 W Mulberry Place, Denver, CO: On and preceding 9/24/18, the employer did not ensure that each affected employee use protective footwear when working in areas where there is a danger of foot injuries due to falling or rolling objects, or objects piercing the sole, and where such employee's feet are exposed to electrical hazards. Employees operated Nissan and Komatsu powered industrial trucks to remove engines from automobiles and to transport parts through the facility's yard and to storage racks.  The powered industrial trucks utilized attachments to, among other operations, facilitate removal and transport of engines from automobiles.  The attachments were frame parts from automobiles that the employer recycled.  The employer did not receive written approval from the powered industrial truck manufacturers.  This condition exposed employees to struck-by hazards.
