29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about 9/16/18, the employer failed to ensure that the following machinery had machine guarding methods in place to protect employees from ingoing nip points:  A) The guard on the floor of the MHI Single Facer #s 1-4 Roll Dollies were not secured, and B) The Corrugator wet end did not have guarding in place on the rollers.   Westrock, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1177572, citation number 1 item number 2 and was affirmed as a final order on 12/5/2016, with respect to a workplace located at 975 N. Freedom St. Ravenna, Ohio 44266.
