29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:     At a residential construction site, employees performing framing work were not provided a protective helmet while working underneath employees framing above at higher levels. The employees were exposed to being struck by objects and/or tools falling from a height greater than (8 to 15) feet.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):      At a residential construction site, employees performing framing activities were exposed to an unprotected fall hazard of approximately 8 feet to the ground below.      Jose Luis Maldonado-Puente was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6-feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of '1926.502(k) developed and implemented, which was contained in OSHA inspection number 1091182, citation number 1, item number 1 and 2 was affirmed as a final order on March 22, 2016 with a final abatement date of  March 24, 2016 with respect to a workplace located at 696 Fossil Wood Drive, Saginaw, Texas 76179.
