29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed: a) On or about September 25, 2018, the employee was exposed to the hazard of falling from a portable fiberglass A-frame stepladder. The employee was working from a Louisville stepladder that was leaning against the side of the building. The stepladder was not fully opened and locked in place per the manufacture guidance and the American National Standards Institute 14.5. Employee was using a Louisville 8 foot stepladder. b) On or about September 25, 2018, the employee was exposed to the hazard of falling from a portable fiberglass A-frame stepladder. The employee was working from the ladder with one foot on the ladder and the other foot on a cement bollard. The employees body was not center between the side of the ladder per the manufacture guidance and the American Nationals Standards Institute 14.5 Employee was using a Werner 8 foot stepladder.
