Site - On or about July 31, 2018, the Werner 24 foot extension ladder utilized by employees to access the roof area did not extend 3 feet above the upper landing surface. Instance b. Site - On or about July 31, 2018, the 16 foot extension ladder utilized by employees to access the roof area did not extend 3 feet above the upper landing surface. Instance c. Site - On or about July 31, 2018, the 30 foot extension ladder utilized by employees to access the upper roof area did not extend 3 feet above the upper landing surface.
