29 CFR 1910.1200(e)(1): Employer had not developed or implemented a written hazard communication program included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii): a). On or about 5/22/18, on site, for employees working at the GL&V USA, Inc site. Employees working with cleaning products such as EnvirOx H2Orange2 Concentrate 117. The spray contains chemicals such as, but not limited to, Hydrogen Peroxide (3.75-4.15%) and Orange Oil (<2%). The SDS states that exposure may potentially cause serious eye irritation. The employer did not implement a hazard communication program to include a written program and training. b). On or about 5/16/18, on site, for employees working at the Saratoga State Park-Roosevelt Spa Bath. Employees working with cleaning products such as EnvirOx H2Orange2 Concentrate 117. The spray contains chemicals such as, but not limited to, Hydrogen Peroxide (3.75-4.15%) and Orange Oil (<2%). The SDS states that exposure may potentially cause serious eye irritation. The employer did not implement a hazard communication program to include a written program and training.

On or about 5/10/18, on site, for employees working at the GL&V USA, Inc. site. Employees were provided with a 3M N95 dust mask on a voluntary basis. The employer did not provide training as outlined in the Appendix D of the standard.
