29 CFR 1926.502(d)(17): The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body narness was not located in the center of the wearer's back near shoulder level, or above the wearer's head.    On or about 9/26/2018 at 4472 Orchard Grove, Lot 11, Orchards at Mill Creek subdivision, Auburn, GA: the employer failed to ensure that roofers installing paper and shingles were protected against fall hazards where workers were observed wearing rock climbing harnesses. Workers were exposed to up to 24-foot fall hazards from the 14/12 pitch roof.
