Employees were working at heights greater than 6 feet without fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At the jobsite located at 17629 W. 83rd Terr., Lenexa, KS, employees using the portable extension ladder to access the roof were exposed to fall hazards of up to approximately 11 feet, 4 inches in that the side rails of the ladder extended approximately 18 inches above the landing surface of the roof.
