29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a)  On or about September 25, 2018, at the commercial construction site, where employees were exposed to fall hazards above 6-feet while working from the 6:12 pitched roof of the building without the use of a guardrail system or personal fall arrest system.

The program did not enable each employee to recognize the hazards of falling and did not train each employee in the procedures to be followed in order to minimize these hazards:  a)  On or about September 25, 2018, at the commercial construction site, where employees were the employer did not train employees on fall hazards associated with working 6-feet above the ground level below on roofs greater than a 4:12 pitch.
