29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure. Maintenance shop, adjacent to roll-up door; on or about 26 September 2018 and times prior thereto; an energized 25-foot blue extension cord was substituted for permanent electrical wiring. Extension cord was attached to wall and routed to the ceiling to provide power for a security camera. This condition exposed employees to electrical shock and electrical burn hazard.

29 CFR 1917.151(g)(8): Abrasive wheels with operational defects were used. Maintenance shop, on or about 26 September 2018 and time prior thereto, an 8-inch Heavy Duty DeWalt Bench Grinder, Model DW758, serial number 2007-07-208; had a damage abrasive wheel. The abrasive wheel had not been properly dressed. Abrasive wheel had a groove of approximately 1/8 of an inch in depth. This condition exposed employees to a stuck by hazard.

29 CFR 1917.151(h)(1): Rotating parts such as gears and pulleys located 7 feet (2.13 m) or less above working surfaces were not guarded to prevent employee contact with moving parts. Receiving shack, on or about 26 September 2018 and time prior thereto, a Q-Mark electrical heater, model MUH0521, the periphery fan blades were not guarded with protective barrier. The measured distance from the floor to the fan blade was 68 inches. This condition exposed employees to a caught in hazard.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket). Tool Shack, on or about 26 September 2018 and times prior thereto; a 7 1/4 inch Worm Drive Skilsaw, model HD77M, serial number HG-093628 was not visually inspected. The outer protective insulation jack had visible damage. This condition exposed employees to electrical shock and electrical burn hazards. Alaska Marine Trucking LLC., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.334(a)(2)(i) which was contained in OSHA inspection number 1187333, citation number 1, item number 1(e) and was affirmed as a final order on 28 December 2016, with respect to a workplace located at Alaska Marine Trucking LLC located at Juneau, AK.
