29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: On or about September 26, 2018, an employee was working below other employees on the scaffolding and was not wearing head protection. The employees were bricking a wall on the car wash. The employee was exposed to the hazard of being struck-by falling bricks and tools.

29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights: On or about September 26, 2018, the employees were working from scaffolding that was not fully planked. The employees were bricking the side of a car wash. Employees exposed to a fall hazard of approximately 7 feet and 1 inch high.
