29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed. The employer is failing to protect employees from electrical and crushing hazards. This was most recently observed on September 24, 2018 at the facility located at 805 Crown Pointe Avenue in Omaha, Nebraska. The employer is not ensuring periodic reviews of the lockout/tag out specific procedures are accomplished as outlined in the standard. Lock-out/tag-out is required and performed by employees who are maintaining the Mail Inserters - Pitney Bowes APS Series. The employer has developed written specific procedures as part of the written lock-out/tag-out program; however, the procedures have not been formally reviewed on an annual basis as outlined in this subpart.
