29 CFR 1926.451(e)(9)(i): The employer did not provide safe means of access for each employee erecting or dismantling a scaffold: a) Front of Structure, Right Side - Workers assembling a tubular welded frame scaffold were climbing the scaffolding frames to access the highest platform level, approximately 26 feet above ground, on or about, 9/26/2018. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.451(g)(2): The employer did not provide fall protection for employees erecting or dismantling supported scaffolds: a) Front Center, Top Level of Scaffolding - A worker on an unguarded platform was assembling a tubular welded frame scaffolding system without fall protection, exposed to a 26 foot fall hazard, on or about 9/26/2018. b) Front Right Side, Top Level of Scaffolding - A worker was moving between the scaffolding system on an unguarded platform without fall protection, exposed to a 26 foot fall hazard, on or about 9/26/2018. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.
