29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by ingoing nip points.  a) Line No. 1, Laminator - On or about September 19, 2018, machine operators were required to perform various tasks, including, but not limited to, checking for moisture on the Melsheet, and/or removing pieces of the Melsheet to perform a quality check.  Machine operators were required to work adjacent to the in-running side of the two rotating pinch rolls that were not guarded while performing these tasks. Employees were thereby exposed to the hazards associated with rotating parts.
