29 CFR 1910.23(c)(9): Portable ladders used on slippery surfaces were not secured and stabilized.  a) Rear of structure: Employees engaged in power washing were exposed to fall hazards while working on portable extension ladders that were set up on a wet slippery surface that were not secured and stabilized. On 9/25/2018, an employee fell approximately 16 feet when the ladder slipped while the employee was climbing.

29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:  a) Exterior Rear of structure: Employee engaged in power washing was exposed to fall hazards of approximately 10 feet while working on a roof without using a guard rail system, safety nets, personal fall arrest system, or other feasible means of fall protection, on or about 9/25/2018.
