29 CFR 1926.451(f)(7): The scaffold was erected, moved, dismantled, or altered without the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling, or alteration: (a) Deep Woods Carpentry LLC, at 3131 W Mexico Avenue, Denver, CO: On or about 9/20/18 and preceding, the employer did not ensure that the scaffold was erected, moved, dismantled, or altered with the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling, or alteration. An employee used an Alum-A-Pole Corporation pump jack scaffold to install windows at an apartment complex of three story buildings. A guardrail system was not installed on the open side and ends of the platform. The employer did not ensure that the scaffold was erected under the supervision of a competent person. This condition exposed the employee to an approximate nineteen foot fall hazard.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms: (a) Deep Woods Carpentry LLC, at 3131 W Mexico Avenue, Denver, CO: On or about 9/20/18 and preceding, the employer did not ensure that a guardrail system was installed along all open sides and ends of the platform. An employee used an Alum-A-Pole Corporation pump jack scaffold to install windows at an apartment complex of three story buildings. A microlam board was used as a top rail. A guardrail system was not installed on the open side and ends of the platform. This condition exposed the employee to an approximate nineteen foot fall hazard.
