29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: a) On or about September 7, 2018, an employee (truck driver) making a delivery to the JR Short Milling Company dba JR Short Snack Products facility in Kankakee, Illinois experienced an amputation of the first digit of the right index finger. The employer did not contact OSHA within 24 hours after an employee sustained a work-related amputation.
