29 CFR 1926.451(e)(6): Integral prefabricated scaffold access frames did not meet the requirements of paragraphs (e)(6)(i)-(vi) of this section: The employer is failing to protect employees from fall hazards associated with using integral prefabricated scaffold access frames during stucco installation preparation and stucco installation. This was most recently documented on or about September 26, 2018 at the residential construction site, located at 109 Carson St, Lees Summit, Missouri. The Integral scaffold access were not specifically designed and constructed for use as ladder rungs in that rungs were not all at lease eight inches in length and uniform.

This was most recently documented on or about September 26, 2018 at the residential construction site, located at 109 Carson St, Lees Summit, Missouri. The tubular prefabricated scaffold frames were not equipped with guardrails and employees wore no personal fall arrest system to protect from approximate 13 foot fall hazards.

An employee wearing no personal fall arrest system was working from a roof overhang above the garage while taping plastic over a window. The employee was expose to fall hazard greater than 6 foot.
