29 CFR 1926.501(b)(15): Each employee on a walking/working surface 6 feet (1.8 meters) or more above lower levels was not protected from falling by a guardrail system, safety net system, or personal fall arrest system: On or about October 4, 2018 and at times prior to employees were in the process of installing electrical wiring on a flat concrete surface were utilizing a scissor lift to access the ceiling tile were not tied off when they were exposed to fall of approximately 28 feet to the concrete below.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. On or about October 4, 2018, the employee failed to report a hospitalization within the 24 hour time period: a) On or about July 27, 2018, Electrician - An employee installing electrical wiring fell from approximately 28 feet to the concrete below resulting in admittance to the hospital for treatment. The employer did not report a hospitalization to OSHA within 24-hours as required.
