29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: (a) Location - 7 Abbot Road, North Reading, MA: On or about 9/21/2018, the main electrical service control panel of a single family home was removed from the wall during demolition while energized and remained energized on the floor as employees continued demolition operations in the area.

29 CFR 1926.850(a): An engineering survey was not performed by a competent person to determine the conditions of the framing floors and walls and the possibility of unplanned collapse of any portion of the structure prior to permitting employees to start demolition operations. (a) Location - 7 Abbot Road, North Reading, MA: On or about 9/21/2018, manual demolition of the celling and back wall of the second story of a single family home began before a survey was made by a competent person to determine the possibility of and the steps necessary to prevent the unplanned collapse of the home.
