29 CFR 1926.1203(a): Before it began work at a worksite, the employer did not ensure that a competent person identified all confined spaces in which one or more of the employees it directed may have worked, and/or did not identify each space that was a permit space, through consideration and evaluation of the elements of that space, including testing as necessary: (a) Location - 100 Hanover St., Boston, MA On or about September 26, 2018, an employee was working in brick lined vault under the street installing ceiling panels to keep the ceiling bricks from falling. The space met the definition of a permit required confined space (PRCS) and it had not been designated as a PRCS by the employer or a designee.
