29 CFR 1926.502(i)(3): All covers were not secured when installed so as to prevent accidental displacement by the wind, equipment, or employees: (a) Neenan Archistruction at 1056 E 19th Ave. Denver, CO 80218: On or before 9/28/18, the controlling employer did not protect employees and subcontractor employees from injury in that a hole approximately 12 inches by 18 inches exposing the next lower level was covered with plywood but not secured. This condition exposed employees to a tripping hazard as well as a fall hazard of approximately 10 feet.

29 CFR 1926.1053(b)(13): The top and top step of a stepladder was used as a step: (a) Neenan Archistruction at 1056 E 19th Ave. Denver, CO 80218: On or before 9/28/18, the controlling employer did not protect employees from injury in that the top step of a step ladder was used to access the raised platform for the elevator. This was the only means of access made available for this platform. This condition exposed employees to a fall hazard of approximately 3 feet.
