29 CFR 1926.502(i)(3): All covers were not secured when installed so as to prevent accidental displacement by the wind, equipment, or employees: M.P. Contracting at 1056 E 19th Ave. Denver, CO 80218: On or before 9/28/18, the creating, correcting and exposing employer did not protect employees from injury in that a hole exposing the next lower level was covered with plywood but not secured. The measured approximately 12 inches by 18 inches. This condition exposed employees to a trip hazard as it was located in a common path of travel.
