29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) Left side of Aquafil USA Building: On or about September 28, 2018 and at times prior, the employer exposed employees to fall from height (approximately 32 feet), in that an adequate training program was not provided to employees so that they could recognize fall hazards and follow procedures to minimize them before being expected to perform tasks associated with erecting of steel members while in the basket of an aerial lift.
