29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  Jobsite:  The employees engaged in setting up a residential home from the second (2nd) floor to install roof trusses were not protected from the hazard of falling approximately 17 feet 10 inches to lower level through floor holes.    On 09/26/2018, one of employees performing the above task fell through a floor opening and sustained multiple fractures from the incident.
