29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) On or about September 24, 2018, and at times prior, Giovanni Cruz Hernandez dba roofing and gutter professionals as a controlling employer did not ensure that employees conducting re-roofing operations at 1365 Peninsula Cir., in Castle Rock, CO 80104 were protected from falls greater than 25 feet resulting in an employee falling from the roof and sustaining severe injuries.  Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").  Roofing & Gutter Professionals, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501 Duty to have fall protection, which was contained in OSHA inspection number 1328171, citation number 1, item number 2 and was affirmed as a final order on August 6, 2018, with respect to a workplace located at 1407 N. Murray Blvd., Colorado Springs, CO 80915.
