29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Special Processes Department, Spray Dryer #2. On or about September 25, 2018 through October 2, 2018, spray dryer operator(s) performed servicing and maintenance tasks during setup such as adjusting/changing BFM fittings and cleaning moisture from BFM fittings and were exposed to amputation and laceration hazards when their hands were near the moving parts of a rotary airlock valve. The employer did not develop, document and require spray dry operators to utilize machine-specific lockout/tagout procedures for the control of hazardous energy sources on Spray Dryer #2. Alternative protective measures, such as machine guarding, were not adequate. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(d): The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: Special Processes Department, Spray Dryer #2. On or about September 25, 2018 through October 2, 2018, spray dryer operators who performed servicing and maintenance tasks during setup such as adjusting/changing BFM fittings and cleaning moisture from BFM fittings were exposed to the moving parts of a rotary airlock valve. Employees were not required to control hazardous energy sources in the following sequence: 1) turn off and shut down the machine, 2) physically locate and operate energy isolating devices in such a manner as to isolate the machines or equipment from the energy source(s), 3) affix lockout or tagout devices to each energy isolating device, 4) verify that isolation and deenergization of the machines or equipment have been accomplished. Employees were exposed to amputation and laceration hazards from moving parts that were not deenergized. Alternative protective measures, such as machine guarding, were not adequate. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

On or about September 25 through October 2, 2018, spray dryer operators who performed servicing and maintenance activities such as removal and replacement of BFM components and cleaning of moisture from BFM components were exposed to amputation and laceration hazards when their hands were near the moving parts of a rotary airlock valve. The employer did not ensure that lockout/tagout training was provided to all authorized and affected employees who were assigned such tasks, to ensure that the purpose and function of the energy control program were understood by employees and that the knowledge and skills required for the safe application, usage and removal of the energy controls were acquired by employees. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Special Processes Department - On or about September 25, 2018 through October 2, 2018, employees were exposed to fractures if caught in rotating parts on the following machines: a) Pre-Mix Color Paste Mixing Tank #T31-100. Employees added product by hand with the mixing blades running. The tank cover was not affixed or interlocked, and did not have a protected area for adding product, such as a chute. b) Pre-Mix Color Paste Mixing Tank #T30-250. Employees added product by hand with the mixing blades running. The tank cover was not affixed or interlocked, and did not have a protected area for adding product, such as a chute. c) Spray Dryer #2. The external rotating parts on both ends of the rotary airlock valve were not guarded. d) Spray Dryer #2 - Adequate guarding was not provided around the viewport opening and BFM fitting to prevent employee access to the moving parts of the rotary airlock valve. Abatement documentation is required for this item in accordance with the requirements of 29 CFRF 1903.19(d).
