29 CFR 1910.119(j)(4)(iii): The frequency of inspections and tests of process equipment were not consistent with applicable manufacturers' recommendations and good engineering practices, and more frequently if determined to be necessary by prior operating experience: a) Utah Gas Corp, 3606 County Road 116, Rangely, CO 81648: On and before September 25, 2018, in accordance with manufacturers' recommendations and good engineering practices, a mechanical inspection of the piping system was due 8/5/2012, but was not yet completed. This condition exposed employees to fire and explosion hazards. Abatement Note: One feasible and acceptable means of abatement includes following API 570 - Piping Inspection Code: In-service Inspection, Rating, Repair, and Alteration of Piping Systems, Section 6.3, Table 1, which provides for frequencies of inspections for thickness measurements and visual external inspection for classes of piping. This Table provides that the inspection interval for Class 1 piping is 5 years for both thickness measurements and visual external inspections, and for Class 2 piping, the inspection interval stated is 10 years for thickness measurements, and 5 years for visual external inspections. Abatement Note: The 2018 PSM Audit dated 4/30/18 - 5/3/18 also provides that documentation was not found to indicate facility process piping has undergone inspection at the frequency defined by selected codes and standards.
