29 CFR 1910.119(j)(4)(iii): The frequency of inspections and tests of process equipment were not consistent with applicable manufacturers' recommendations and good engineering practices, and more frequently if determined to be necessary by prior operating experience:    a)  Utah Gas Corp, 3606 County Road 116, Rangely, CO  81648:  On and before September 25, 2018, in accordance with manufacturers' recommendations and good engineering practices, a mechanical inspection of the piping system was due 8/5/2012, but was not yet completed.  This condition exposed employees to fire and explosion hazards.     Abatement Note:  One feasible and acceptable means of abatement includes following API 570 - Piping Inspection Code:  In-service Inspection, Rating, Repair, and Alteration of Piping Systems, Section 6.3, Table 1, which provides for frequencies of inspections for thickness measurements and visual external inspection for classes of piping.  This Table provides that the inspection interval for Class 1 piping is 5 years for both thickness measurements and visual external inspections, and for Class 2 piping, the inspection interval stated is 10 years for thickness measurements, and 5 years for visual external inspections.    Abatement Note:  The 2018 PSM Audit dated 4/30/18 - 5/3/18 also provides that documentation was not found to indicate facility process piping has undergone inspection at the frequency defined by selected codes and standards.

29 CFR 1910.119(l)(1): The employer did not establish and implement written procedures to manage changes (except for "replacements in kind") to process chemicals, technology, equipment, and procedures; and, changes to facilities that affect a covered process:    a)  Utah Gas Corp, 3606 County Road 116, Rangely, CO  81648:  On and before September 25, 2018, the employer did not establish management of change procedures to ensure that the impact of changes were addressed prior to any changes for process chemicals, technology, equipment, procedures, and changes to facilities that affect a covered process, such as, but not limited to, changing out and bypassing the connector for the Ethyl Propyl Butane Condensate (EPBC) to the Natural Gas Tank (NGT) in the storage tank area.  This condition exposed employees to fire and explosion hazards.

29 CFR 1910.119(l)(2)(ii): 29 CFR 1910.119(l)(2)(ii):  The management of change procedures did not assure that the impact of changes on safety and health were addressed prior to any change:  a)  Utah Gas Corp, 3606 County Road 116, Rangely, CO  81648:  On and before September 25, 2018, the employer changed out and bypassed the connector for the Ethyl Propyl Butane Condensate (EPBC) to the Natural Gas Tank (NGT) in the storage tank area without preparing a management of change to ensure that the impact on safety and health was addressed prior to the change.  This condition exposed employees to fire and explosion hazards.

29 CFR 1910.119(n): The employer did not implement an emergency plan for the entire plant in accordance with the provisions of 29 CFR 1910.38:    a)  Utah Gas Corp, 3606 County Road 116, Rangely, CO  81648:  On and before September 25, 2018, the employer did not establish and implement an emergency action plan for the entire plant in accordance with the provisions of 29 CFR 1910.38.  The employer did not have an alarm system with a distinctive signal for facility equipment hazards such as fires, overpressures, and releases, and did not comply with the requirements in 1910.165.  This condition exposed employees to fire and explosion hazards.

The compilation of written process safety information is to enable the employer and the employees involved in operating the process to identify and understand the hazards posed by those processes involving highly hazardous chemicals.  This process safety information did not include information pertaining to the equipment in the process, including piping and instrument diagrams (P&ID's):    a) Utah Gas Corp, 3606 County Road 116, Rangely, CO  81648:  On and before September 25, 2018, the employer did not complete a compilation of written process safety information before conducting any process hazard analysis required by the standard.  This process safety information did not include information pertaining to updated piping and instrument diagrams (P&ID's) since Utah Gas Corporation acquired the site in approximately March, 2107.  This condition exposed employees to fire and explosion hazards.

29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not conducted at least once every three years:  a) Utah Gas Corp, 3606 County Road 116, Rangely, CO  81648:  On and before September 25, 2018, the employer did not conduct an evaluation of each powered industrial truck operator's performance at least once every three years.  Employees' forklift training certifications expired between 2014-2017.  This condition exposed employees to the hazard of being struck-by forklifts.
