On or about October 2, 2018, at the above addressed jobsite, an employee using a radial power saw did not wear safety glasses, thereby exposing the employee to the hazards associated with eye injuries.

On or about October 2, 2018, at the above addressed jobsite, employees performing residential roofing operations on a roof seventeen (17) feet six (6) inches above the ground were not protected from falls, thereby exposing employees to fall hazards.
