29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  Valerie Burgess dba Bay View Renovations, LLC; The employer does not require the use of fall protection during roofing operations. This violation was most recently observed at a residential re-roofing project located at 3012 Lakeshore Way in Twin Lakes, WI, where four (4) employees were not protected from fall hazards of approximately 19.3 feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a)  Valerie Burgess dba Bay View Renovations, LLC; The employer does not require that employees exposed to fall hazards be trained to recognize and minimize those hazards. This violation was recently observed at a residential re-roofing project at 3012 Lakeshore Way in Twin Lakes, WI, where at least one (1) employee had not been provided fall protection training.   The Valerie Burgess dba Bay View Renovations, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 1099638, citation number 2, item number 2 and was affirmed as a final order on January 15, 2016, with respect to a workplace located at 7850 East Wind Lake Road, Wind Lake, WI 53185.  The Valerie Burgess dba Bay View Renovations, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 1188321, citation number 2, item number 2 and was affirmed as a final order on May 18, 2017, with respect to a workplace located at 1703 S. Miller Parkway, Milwaukee, WI 53214.
