This violation was recently observed at a residential re-roofing project at 3012 Lakeshore Way in Twin Lakes, WI, where at least one (1) employee had not been provided fall protection training. The Valerie Burgess dba Bay View Renovations, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 1099638, citation number 2, item number 2 and was affirmed as a final order on January 15, 2016, with respect to a workplace located at 7850 East Wind Lake Road, Wind Lake, WI 53185. The Valerie Burgess dba Bay View Renovations, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 1188321, citation number 2, item number 2 and was affirmed as a final order on May 18, 2017, with respect to a workplace located at 1703 S.
