29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about October 3, 2018, the worker was installing tape around the upper windows that were protruding from the roof top. The worker was exposed to fall hazard of approximately 12 feet to the ground below.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed: On or about October 3, 2018, the employee was exposed to the hazard of falling from a 4 feet fiberglass A-frame step ladder. The employee was working from the stepladder that was leaning against the side of the building and at times leaning on the glass door. The stepladder was not extended out and locked in place per the manufacture guidance and the American National Standards Institute 14.5. Employee was using a 4 foot stepladder.
