29 CFR 1928.57(a)(6)(iii): Instruction was not provided to stop engine, disconnect the power source, and wait for all machine movement to stop before servicing, adjusting, cleaning, or unclogging the equipment.    a) Facility: On October 2, 2108 and at times prior thereto the energy control training provided by the employer was not clear or consistent in identifying when an energy control device is needed when performing servicing and maintenance activities to include cleaning and clearing jams.          b) Facility: On October 2, 2108 and at times prior thereto the energy control training provided by the employer did not clearly identify the roles and responsibilities between mechanics and mechanics helpers. 29 CFR 1928.57(a)(11): Where a moving machinery part presented a hazard during servicing or maintenance, the engine was not stopped, the power source was not disconnected, and all machine movement was not stopped before servicing or maintenance was performed, except where the Employer could establish the following in 1928.57(a)(11)(i-iii):    a) Facility: On October 2, 2018 and at times prior thereto the mechanics helpers were not protected by their own personal lockout device when performing scheduled or unscheduled maintenance activities along with the head mechanics. This included but may not be limited to scheduled and unscheduled maintenance on the grizzly, picker or chopper machines.        Note) Certification of corrective action is required for this item.

29 CFR 1928.57(c)(2)(i): The mesh or nip-points of all power driven gears, belts, chains, sheaves, pulleys, sprockets, and idlers were not guarded:    a) Picker machine building: On October 2, 2018 and at times prior thereto employees were exposed to caught-by hazards when performing cleaning and "trash clearing" activities in and around conveyor systems that include, but may not be limited to, the conveyor system located under the "grizzly" chains on the north picker.       b) Picker machine building: On October 2, 2018 and at times prior thereto employees were exposed to caught-by hazards when observing machine operations and performing cleaning activities near an unguarded belt and pulley system in the "thinner" area of the north picker.        Note) Certification and documentation of corrective action are required for this item.
