29 CFR 1910.30(a)(1): Before any employee is exposed to a fall hazard, the employer did not provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart: Loading Area: On or about October 4, 2018, an employee was exposed to a fall hazard of greater than 4 feet while loading silica sand into the hatch of a semi-trailer without being properly trained in the use of fall protection equipment.

29 CFR 1910.219(d)(1): Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, were not guarded in accordance with the standards specified in paragraphs (m) and (o) of this section: Production Plant side: On or about October 4, 2018, and at times prior thereto, employees were exposed to a caught-in hazard while shoveling sand adjacent to a pulley that had the guard removed. Arepet Industries, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.219(e)(2)(i) which was contained in OSHA inspection number 1122374, citation number 1, item number 2, and was affirmed as a final order on May 31, 2016, with respect to a workplace located at 3603 Speedway Run, Von Ormy, TX 78073.
