29 CFR 1910.28(b)(4)(i): The employer must ensure that each employee on a dockboard is protected from falling 4 feet (1.2 m) or more to a lower level by a guardrail system or handrails. On or about October 03, 2018, one employee was exposed to a fall greater than four feet while moving donated items from the dock lift to and from the delivery truck.

29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. On or about October 04, 2018, there wasn't a written energy control program for employees to follow when de-energizi ng the dock lift to clean beneath the dock lift.
