29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:    a. On or about September 25, 2018, at the workplace located at 8800 Guernsey Industrial Boulevard, Cambridge, Ohio, at the Line 1 Lyle Industries Trimmer, serial number GM3381, the employer did not ensure employees were protected from the unexpected start-up of electrically-driven pneumatic assist mold press and release of stored energy of pneumatic guillotine cutters within the machine when removing jams, thereby exposing employees to caught-in/caught-between hazards.   As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lock-out/tag-out device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)], were not implemented to protect employees from the unexpected energization, startup, or release of stored energy during machine servicing and maintenance activities.        On September 25, 2018, an employee reached into the confines of the Line 1 Trimmer Reamer in order to remove a jam by hand when the mold press cycled causing crushing injuries to the fingers. The integrated safety system for the mold press was not control reliable resulting in the unexpected cycle of the equipment.
