29 CFR 1926.304(d): When the tool was withdrawn from the work, the lower guard did not automatically and instantly return to the covering position. On the east side of the construction site, employees using skill saws to cut framing material had the lower saw guard wedged in the up position exposing the employees to amputation hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). On the east side of the construction site, employees engaged in framing activities without fall protection were exposed to a fall hazard of approximately 9 and 18 feet to the ground below.
