29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws. On the west side of the construction site, employees engaged in framing activities utilized an extension cord that had the outer sheathing pulled away exposing internal wiring were exposed to electrical shock hazards.

On the west side of the construction site, employees engaged in framing activities utilized an extension cord that was missing the ground pin to operate Makita skill saws exposing employees to electrical shock hazards. Luis Daniel was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.404(f)(6), which was contained in OSHA inspection number 1300935, citation number 2, item number 1/a and was affirmed as a final order on 04/20/2018, with respect to a workplace located at 7415 Seclusion Ridge Dr., Arlington, Texas 76001.
