29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). On or about October 1, 2018, a sub-contractors employees were exposed to a cave in while performing masonry activities on a new commercial building located at 4173 Westford Place, Canfield, Ohio 44406. The sub-contractors employees were constructing a foundation wall in an excavation that was approximately 12 feet deep and the employer did not ensure that they were using cave in protection.
