On or about October 4, 2018, at the above addressed jobsite, employees conducting carpentry operations, drywall operations and painting operations on the third floor, were exposed to injuries from stepping into holes measuring nineteen (19") inches in length by twelve (12") inches width. The depth was approximately two (2) to three (3) inches in depth.

On or about October 4, 2018 - at the above addressed jobsite, employees were lifting paint buckets using a manufactured box that was damaged on a JLG Rough Terrain Forklift Model #G12-55A TURB. The box was heavily damaged and rigged improperly to the fork mast with electrical wire. This practice could affect the capacity and the safe operation of the equipment and the manufacturers written approval was not obtained, thereby exposing the employees to fall and struck by hazards.
