OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or were likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards as an employee was operating a powered industrial truck (PIT) in a warehouse and was not wearing a seat belt. In the loading area at 32 Howe Avenue Millbury, MA 01527: On October 4, 2018, an employee was operating a PIT and was not wearing a seat belt.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:. United County Industries Corp., 32 Howe Ave., Millbury, Massachusetts; On and prior to October 4, 2018 extension cords were used in lieu of permanent wiring in the facility. a. In the QC/Shipping area - several flexible cords and a relocatable power tap were used to power a light and portable tools in lieu of permanent wiring. The tool was attached to the building with a steel cable and there was a 4 outlet box in the vicinity that could have been used to provide power. b. At the wash tank there was a single flexible cord, covered with oil, that was used in lieu of permanent wiring for the sump pump and oil scraper. United County Industries Corp. dba County Heat Treat was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.305(g)(1)(iv)(A), which was contained in OSHA inspection number 1133796, citation number 1, item number 3 and was affirmed as a final order on 4/15/2016, with respect to a workplace located at 32 Howe Ave., Millbury, Massachusetts.
