29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) MM Packaging Products, Inc, located at 3915 N. Garfield Ave., Loveland, CO 80538: On and preceding 4 October, 2018, the employer did not ensure that procedures were developed for the control of potentially hazardous energy when employees were engaged in maintenance, cleaning and tooling changes on equipment with multiple power sources. Employees perform maintenance, tooling changes and cleaning of a Hendrick Sierra 3700W beam saw. During maintenance, blade changes and servicing, unexpected release of stored energy could occur due to the pneumatic power, used for several functions of the saw, not being isolated. The following condition exposed employees to caught-in hazards.

Garfield Ave., Loveland, CO 80538: On and preceding 4 October, 2018, the employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed. Employees perform maintenance, tooling changes and cleaning of Hendrick beam saw. The energy control program provided by the employer showed a procedure for periodic inspection of the Hazardous energy control program, however, no documentation was provided to indicate that inspections have been completed. The following condition exposed employees to caught-in hazards.
