29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury: a) Job site, 606 Cedar Avenue, Pittsburgh, PA: On or about October 2, 2018, during construction activities, the employer did not ensure that employees were provided with basic safety and health training relevant to their work environment. Deficiencies noted included, but were not limited to, failing to provide safety training on personal protective equipment where head protection was needed, not providing instruction on electrical safety where hazards such as inappropriate electrical splices, lights without covers and openings in temporary electrical service circuit breaker panels existed and not ensuring that employees were aware of the need for handrails on stairs.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards that enabled each employee to recognize the hazards of falling and that trained each employee in the procedures to be followed in order to minimize these hazards: a) Job site, 606 Cedar Avenue, Pittsburgh, PA: On or about October 2, 2018, during construction activities, the employer did not ensure that an employee was provided with appropriate fall protection training where fall hazards of 20 feet or greater existed.
